Job summary

We are looking for a dedicated and passionate Health & wellbeing Coach delivering on our One You Haringey smoking cessation services, you will play a vital role in helping individuals improve their overall health and well-being.

The role will include:

  • Delivering stop smoking clinics and offering person-centred support to residents who want to quit smoking.

You must be passionate about helping residents stop smoking and live a healthier life whilst making a difference in the community. Join us in our mission to empower individuals to break free from living an unhealthy lifestyle and to make the behaviour changes necessary to improve health outcomes.

Main duties of the job

  • Deliver personalised, one to one behavioural support to individuals wishing to stop smoking, assessing readiness to quit and tailoring interventions to meet individual needs.
  • Providing ongoing support and encouragement to participants throughout their journey with the service.
  • Delivering stop smoking sessions face to face within community settings, and remotely by telephone.
  • Monitor, review and evaluate participant progress against agreed outcomes, adjusting support plans as required.
  • Collaborating with other healthcare professionals and community organisations to promote the One You Haringey programme.
  • Maintain accurate, timely and confidential records of client interactions and program outcomes.
  • Liaise with professionals including GPs and primary care staff to promote and generate referrals to the service.
  • Attend community outreach events to promote the service within Haringey.
  • Contribute to the achievement of service targets and performance outcomes.

About us

Haringey GP Federation is a dynamic and growing organisation working in partnership with GP practices across the diverse borough of Haringey, north London. Formed in 2016 by local GPs, we now work alongside over 30 practices within seven Primary Care Networks to help deliver high-quality, accessible healthcare to our local communities.

Our role is to support and enable general practice by developing and delivering services that complement core GP work, creating opportunities for shared learning and innovation, and helping practices respond to the evolving needs of patients and the wider health system.

We work in close partnership with practices, clinicians and system partners to co-design services, pilot new approaches, and build capacity across primary care. Through clinical programmes, workforce development and operational support, our aim is to add value in ways that are practical, respectful and aligned with the priorities of our member practices. Essential

  • A minimum of 2 A-Levels or an equivalent Level 3 Diploma
  • GCSE English Language at a minimum of a Grade B (or equivalent Level 2 qualification). OR achievement of a Level 2 in Literacy Assessment at interview stage.

Desirable

  • Degree in Public Health, Psychology, or related subject
  • NCSCT Level 2

  • Highly organised with an ability to work to targets.
