JOB TITLE: Lead Nurse Long-term Conditions

REPORTS TO: GP Clinical lead and Head of Integrated Services

ACCOUNTABLE TO: GP Clinical lead and Head of Integrated Services

HOURS: Full-time

SALARY: £44,000 to £50,000 for WTE (37.5 hours) subject to experience

CONTRACT TYPE: Permanent

MAIN LOCATION: Working with General Practices in East Haringey, community centres, Federation main office and potentially home visits.

OFFICE LOCATION: 48 Station Road, Wood Green, N22 7TY

Main duties of the job

Haringey GP Federation (HGPF) has set up a service to identify long term conditions in patients in areas of deprivation who may experience healthcare inequalities. These patients will be invited for a health check with a view of identifying and managing Hypertension (HTN), Chronic Kidney Disease (CKD) and Chronic Obstructive Pulmonary Disease (COPD). As a part of this role you may be asked to support wider nursing work, including supporting other respiratory services, or nursing within the integrated services teams or at practice level.

This could be a great opportunity for a General Practice Nurse with relevant experience in managing patients with long-term conditions who is interested in progressing their career.

Ideally the candidate will have:

Experience working at an advanced level, caring for patients with long-term conditions;

Experience of EMIS Web or System One;

Be aware of national guidelines and management of specific patient groups

  • Is a nurse prescriber

  • Experience of leading a Nursing team and/or health care support teams in a healthcare setting
  • Experience in developing and implementing protocols and clinical guidelines
  • Experience in quality initiatives such as clinical benchmarking

  • Experience of leading a team in a multisite organisation
