Job summary

Job Title:Primary Care Nurse (Haringey Integrated Services)

Hours of work:Full time (37.5 hrs)

Remuneration:£37,000-£42,000 per annum (based on prior experience)

Contract length:Permanent

Responsible to: Practice Manager / Service Manager / Clinical Lead

Accountable to: Senior Integrated Services Manager

Line Manager: Integrated Service Manager

Main Location: Working with General Practice in Haringey with some outbound visits to Care Homes, Supported Accommodation or Home Visits.

You may be asked to work on site at any practice within the Haringey Borough at the needs of the integrated service.

Office Location: Haringey GP Federation Offices (48 Station Road, Wood Green, London, N22 7TY).

Main duties of the job

Some of our Integrated Services include:

Long-term conditions in Health Inequalities Service.

Haringey Homelessness in Health Inequalities Service.

Severe Mental Illness Physical Health Checks Service.

Respiratory Diagnostic Hublets.

You will be employed by the Haringey GP Federation on a full-time basis, to work across our Services, with the possibility of developing your role further into specific speciality areas.

Job responsibilities

Haringey GP Federation is supporting member practices across the borough with the management of patients on diverse range of conditions, including long-term conditions, respiratory diagnostic testing (Spirometry) and physical health checks for patients on the Learning disabilities and Severe Mental Health (SMI) Register. The Integrated Services is a vibrant and highly committed team which provides high-quality clinical Interventions, medicine management, physical health checks and care planning for Haringey registered patients.

There is growing evidence of ethnic inequalities in health care, particularly in relation to access to services. It is believed that 20% of health inequalities relate directly to health service provision and there is more we can do to prevent or delay ill health and treat people quicker. There is also evidence that some ethnic minority communities are more disproportionately affected by long-term conditions than others.

The Integrated Services are one of the Haringey GP Federation largest running group of services, bridging the gap between primary care, secondary care, and the voluntary sector. The Integrated Services aim to deliver on key national and local strategies to reduce unwarranted variation, by working with specific cohort of patients to achieve better health outcomes.

  • Demonstrate practical experience of staff/student supervision.
  • Experience of conducting physical health checks
  • Experience of working with patients that need LTC management - including Diabetes, COPD, Asthma, Hypertension and CKD
  • Experience in mental health nursing and risk assessment and development of management plans

Qualifications

Essential

  • Registered Nurse (NMC)
  • First degree in Adult Nursing or Mental Health Nursing

Desirable

  • Qualification General Practice Nursing
  • Evidence of completion of Educator programme (Mentorship or SSSA)
