Line Manager: Lead Outreach Nurse for the HHHIT Service

Hours of work: Full time (37.5 hrs)

Remuneration: £26,500

Contract length: Permanent

Main Location: Wood Green, Mulberry Junction, and variable work throughout Haringey.

Main duties of the job

The Care Navigator will support patients experiencing homelessness to engage with health and social care services and to reconnect them with primary care. Patients in our service have multiple needs crossing various agencies: drugs and alcohol, mental health, social services, housing support, and primary care. Therefore, the role involves working across several sectors to help patients navigate the complex health and social care systems, overcoming access barriers.

You will work within a multi-disciplinary team and allied agencies to provide individual holistic assessments, while signposting and referring patients to appropriate services and activities that may help to promote their health, wellbeing, and independence.

This role includes working with people who may have experienced previous trauma such as sexual violence, domestic violence, substance misuse, mental health, sex working, severe disadvantage, and patients with previous/ongoing convictions. Therefore, it is imperative that trauma-informed care is provided, which includes understanding, recognising, and responding to the effects of all types of traumas. Effective care navigation is crucial to delivering coordinated, patient-centred care and supporting patients in accessing healthcare appropriately.

You will be working to deliver the key objectives of the service: supporting and strengthening access to General Practice for patients experiencing homelessness in Haringey.
