Hours of work:Fulltime (37.5 hours per week Monday to Friday)Part-time (22.5 hours per week Monday to Friday)

Main duties of the job

As a Care Navigator, you will provide support to elderly or socially isolated patients in Haringey as part of a new Digital Inclusion Service. The service involves providers across several sectors: primary care, community health, hospital, social care and the voluntary sector. The aim of the digital exclusion service will be to ensure that nobody is excluded due to lack of digital access, confidence, or skills. This is a person-centred approach that uses signposting and information to help patients and their carers move through the health and social care system as smoothly as possible to address unmet needs.

You will work with various stakeholders and will provide individual support to those who are found to be digitally excluded and direct patients to appropriate services including activities that may help to promote their health, wellbeing, and independence. As a Care Navigator, you will focus on vulnerable patients or those who are in harder to reach communities including but not limited to: people with learning difficulties, mental health problems, substance misuse, homeless or do not have English as their first language.

We provide a range of services in Haringey, including those which are best delivered by Primary Care at scale, namely; integrated services, working with partners in our local borough partnership, together with support services, including back-office support, workforce and training and some shared staffing clinical teams. We also currently manage two local GP practices and lead the North Central London Training hub.
