Job summary
The Advanced Paramedic role will support primary care to deliver patient services as part of an established practice-based team of paramedics in North and West Herefordshire PCN.
To work within our practices as an autonomous, accountable Paramedic providing holistic care for individuals including clinical assessment, diagnosis, management and treatment.
They will work collaboratively as part of a general practice multidisciplinary team to meet the needs of patients, supporting the delivery of the PCN priorities for improved health and care across our geographical area.
The role will exist within a team and will be both varied and diverse with clinical support and mentorship provided to allowthe successful recruit to flourish. The workload will consist of a mixture of urgent home visit assessments, pro-active assessments as part of our well-being team and duty day cover with opportunity to then follow up the patients seen.

Main duties of the job
Key responsibilities will include:
- Assessing and treating patients in their own place of residence and those attending surgery.
- Triage and treat patients, making any necessary referrals to other members of the primary health care team and MDT and/or organise admission to hospital if required.
- Ensure clinical practice is safe and effective and remains within boundaries of competence and acknowledges practitioners own limitations
- Advise patients on maintaining health and wellbeing, and refer to other team members as need arises.
- Communicate at all levels within the multidisciplinary team ensuring effective service is delivered.
- Maintain accurate, contemporaneous healthcare records appropriate to the consultation.
- Ensure evidence-based care is delivered at the highest standards ensuring delivery of high-quality patient care.
- Work within local policies and procedures.
- Participate in the audit process, evaluation and implementing plans and practice change in order to meet patient need.
- Take responsibility for safeguarding children and vulnerable adults and work with other team members to ensure they are protected.
