South & West
Herefordshire Primary Care Network is looking for a Care Coordinator to
join our Health & Wellbeing Team. The
position will work with our Frailty and Neighbourhood Health teams to support
the PCN in offering access to proactive and reactive personalised care and
support, with a particular focus on supporting those patients at highest risk
of suffering from health inequalities.
The PCN is
comprised of six GP practices centred around the market town of
Ross-on-Wye: Alton Street Surgery,
Pendeen Surgery, Fownhope Surgery, Much Birch Surgery, Golden Valley Practice
and The Surgery Kingstone. The role
will be hosted by the Herefordshire GP Federation, Taurus Healthcare, as the
employer.
* This is a Fixed Term position for 18 months *
*Interviews are expected to be held on Thursday 23rd October 2025*
Main duties of the job
- To work closely with the PCN and practice teams
to identify and work with a cohort of people to support their personalised
care requirements, using any available decision support tools.
- To act as the primary point of liaison for the
PCNs multi-disciplinary team meetings, capturing action steps at all
meetings, and ensuring that all agreed activity is completed.
- To contact patients identified for outreach to
arrange for appointments and to promote healthy lifestyle choices, such as
smoking cessation and health checks.
- To support the members of the Frailty and
Neighbourhood Health teams with outreach to identified cohorts of patients.

healthcare assistants, social prescribing link workers, physiotherapists,

support with the aim of providing advocacy, encouraging independence, a healthy
lifestyle, mental wellbeing, and social connectivity. They may be given a caseload of
identified patients and be required to ensure that their changing needs are
addressed by taking into account local priorities, health inequalities and/or
population health management risk stratification.
The
successful candidate will be a caring, dedicated, reliable, person-focussed and
enjoy working with a wide range of people. They will have good written and
verbal communication skills and strong organisational and time management
skills. They will be highly motivated and proactive with a flexible attitude,
keen to work and learn as part of a team and committed to providing people,
their families and carers with high quality support.
