Advanced Nurse Practitioner (Independent Prescriber)

Job summary

Taurus Healthcare have an exciting opportunity for an Advanced Nurse Practitioner.

They will act as an autonomous practitioner working independently in Taurus Healthcare Clinical service areas to assess, diagnose and treat the conditions of patients attending within primary care services and their own homes, initiating direct referrals as appropriate. Provide expert professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence.

The post holder must possess competencies and capabilities across all four pillars of advanced practice

The role: Face to face and telephone consultations to assess and manage patients in general practice and the wider system, implementing agreed management plans, personalised care planning and following approved protocols as appropriate. Demonstrate critical thinking in the clinical decision-making process, including assessment and diagnostic skills, leading to the delivery of safe care for all patients.

* This is a flexible role for up to 20 hours per week, working between 9 am to 6 pm Monday to Friday (in hours) *

Main duties of the job

Clinical

  • Participate in all treatment and preventative healthcare services within the Herefordshire Remote Health Service as delegated and agreed by the supervising physician(s).
  • Recognise the need for adapting the style of consultation required in a remote role.
  • Recommend and explain appropriate diagnostic tests and treatment.
  • Request and interpret results of laboratory investigations when necessary. Review and communicate results where appropriate.
  • Refer patients as appropriate; in primary, secondary or community care. And participate in peer and expert discussion to determine appropriateness of referral
  • Adopt a Making Every Contact Count approach within the confines of a duty remote consultation, ensuring that patients have been dealt with holistically.
  • Fully document all aspects of patient care and complete all required paperwork for legal and administrative purposes. Use SNOMED coding wherever possible.
  • Update the patient record with information for QOF/DRUMS and other practice monitoring and review systems and understand the implication of record keeping for contractual purposes
  • Undertake other activities when needed that are within the post holders clinical competence and which would not otherwise jeopardise the delivery of primary medical services to the patients.
  • Engage with the nursing and non-clinical members of the Primary care team as required, requesting action where appropriate.

The ANP will work providing care for presenting patients from initial history taking and clinical assessment through to the diagnosis, treatment and evaluation of care. They will demonstrate critical thinking in the clinical decision-making process, including assessment and diagnostic skills, leading to the delivery of safe care for all patients. They will work collaboratively with the healthcare team to meet the needs of the patients, supporting the delivery of policy and procedures.

Lead prescribing clinician supporting nursing services and taking overall clinical responsibility for clinical areas where GP is physically not present -GP support available if needed which may be virtual.

Undertake safe clinical decision-making and expert care, including assessment and diagnostic skills and safety netting for patients using face to face (clinic and home visits), telephony and on line consultation platforms

Demonstrate critical thinking in the clinical decision-making process, with the ability to prioritise and triage the needs of patients, accordingly, instigating appropriate investigations or referrals to colleagues and other care providers

Demonstrate highly developed clinical reasoning and decision-making

Follow local and national guidelines and pathways, working to the latest evidence base, and will practise in a reflective manner, completing significant event analyses and audits to evaluate the effectiveness of the care they provide.Support and manage health needs of women presenting for family planning, cervical cytology or sexual health consultation

Use advanced clinical skills to provide education to service users, promoting self-care and empowering them to make informed choices about their treatment.

Prevent inappropriate hospital admissions or attendances by providing/co-coordinating the provision of specialist care by other agencies for the patient in the most appropriate and agreed environment

Work collaboratively with hospital and community colleagues ensuring patients receive the right care in the right place at the right time.

Refer patients to other medical specialities for assessment if deemed appropriate.

Refer patients as appropriate to other members of the multidisciplinary team, secondary care and to other statutory and voluntary organisations including referral for x-ray.

Undertakes and interprets a range of diagnostic tests and routine clinical procedures

Work independently and without direct supervision in the home environment as required

To act as an autonomous practitioner who directs, provides and evaluates the highest standard of evidence based clinical care to patients within their own home, founded on agreed management care plans and protocols
