Join our team as a Wellbeing Care Co-ordinator in North & West Herefordshire Primary Care Network.

Were looking for an enthusiastic Wellbeing Care Co-ordinator to join our friendly team and be the first point of contact for patients needing proactive well-being support across five local practices.

You will work closely with GP teams, Care Coordinators, social prescribers, health professionals, and community groups to help patients get the right support for their needs.

Youll also be involved in local health projects that focus on helping people who may find it harder to access services.

Working alongside our supportive Wellbeing Team, youll help individuals and groups by offering personalised care guiding them to make informed choices that support their health and wellbeing goals.

*This is a part time position working 18.75 hours per week over 2.5 days*

Main duties of the job

Key responsibilities will include:

In this varied and rewarding role, youll help patients access the care and services they need, while supporting wider health and wellbeing initiatives across the PCN.

  • Be the first point of contact for coordination of new referrals to the Wellbeing Team
  • Offer advice and signposting support to patients (and staff)
  • Work one-to-one with patients, helping them take an active role in their care and connect with the right services
  • Support coordination of group-based health programmes, such as managing long-term conditions like diabetes and high blood pressure
  • Get involved in community outreach and public health campaigns to reduce health inequalities
  • Promote the work of the Wellbeing Team across GP practices and local communities
  • Collaborate with a wide range of professionals including GPs, social prescribers, health coaches, dietitians, and safeguarding coordinators

They work with General Practices and their communities where there are identified health inequalities to engage and support people in accessing health and community services

  • Work closely with and understand the different roles within the wider Wellbeing team (Social prescribers, Health and Wellbeing coaches, dieticians, Care Home and Safeguarding coordinators.) and General Practice based staff.

There will be a need to work agilely- within the local community and in practices as well as remotely depending on the requirements of the service.
