Job summary

We have an exciting opportunity for a non-clinical care coordinator to join our Safeguarding team in North & West Primary Care Network.

You will work closely with the Senior Clinical Safeguarding Care Coordinator to ensure a responsive, high-quality safeguarding service for both children and adults. You will also support the PCN Care Home Care Coordinator in protecting our most vulnerable patients.

The role will work across all PCN practices and the community, collaborating with partner organisations to provide coordinated support to patients.

As a key member of the PCN Wellbeing Team, you will work closely with Care Coordinators, Social Prescribers, Health & Wellbeing Coaches, and allied health professionals.

*Interviews are expected to take place during the week commencing Monday 2nd June 2025*

Main duties of the job

The successful candidate will play a key role in coordinating safeguarding information for both children and adults.

You will support effective communication and collaboration across a wide range of teams, including the Primary Care Network, acute services, voluntary and community organisations, the Multi-Agency Safeguarding Hub (MASH), and the wider Integrated Care System (ICS).

Key responsibilities will include:

Providing administrative support for safeguarding and Care Home processes.

Managing safeguarding documents and records with accuracy and confidentiality.

Ensuring clear, timely communication with partner organisations.

Attending meetings with healthcare professionals and statutory agencies, including GPs, Primary care nurses, health visitors, school nurses, and social care teams.

Facilitating multi-disciplinary team working by acting as a liaison between teams involved in the care of vulnerable individuals, such as the Primary Care Network team, Multi-Agency Safeguarding Hub (MASH), and the Integrated Care Board (ICB).

The Care Coordinator for Safeguarding and Care Homes in the N&W PCN will play a key role in supporting the Senior Clinical Safeguarding Care Coordinator and work alongside the existing Care Home Care Coordinator to protect our most vulnerable patients (Adults and Children). The role will:

  • Work within N&W PCN GP Practices to support central co-ordination for safeguarding information, communication and patient care planning.
  • Be GP facing with the core responsibility of maintaining consistent protocols regarding safeguarding information across the network.
  • Oversee safeguarding administration, document handling, record management and communication with partner organisations.
  • Support continuity of care, facilitating smooth and planned discharge and handover between care settings across the health and social care system.
  • Support multi-disciplinary team working and provide assistance with liaison between the Primary Care Network team, acute, voluntary and community services, Multi Agency Safeguarding Hub and the wider ICB.
  • Support audit and continuous improvement of safeguarding practices within the PCN to support adoption of best practice across the PCN.

Main Responsibilities

Safeguarding duties:

1. Responsible for the workflow of safeguarding reports and documents, ensuring they are circulated to appropriate clinicians in line with practice safeguarding policies and document preparation.

2. Summarise and code the documents into the relevant specific templates, highlighting to patients registered GPs anything significant

3. Maintain a register of children on Child Protection Plans, Child in Need plans, Children in Care and those children deemed to cause for concern

4. Ensure the correct coding is added to patient records to aid clinicians in identifying those children and adults who are cause for concern and to add alerts to parent records

5. Contribute to data collection and audits in relation to safeguarding to support and improve standards for safeguarding practice across the PCN.

6. Update alerts and warnings in the Emis record according to practice safeguarding protocols when safeguarding concerns are recorded.

7. Support the coordination and delivery of MDTs within the PCN.

8. Redact and update the Emis record as per practice safeguarding protocols when a patient reaches 18 years of age, under the guidance of the Senior Clinical Care Coordinator.

9. Any other tasks delegated by practices, suitable to the candidates skills and competencies.

Safeguarding Partner Communications

Prioritise and respond to strategy meeting invitations as advised by the Senior Safeguarding Care Coordinator and/or the practice safeguarding lead GP.

Monitor DNAs for children in regard to missed clinical appointments and those children who have not had their routine immunisation. Escalating both to the Senior Clinical Care Coordinator for Safeguarding, in line with practice safeguarding policies.

Follow up on those children who have not had their routine immunisations, as identified by administration with the appropriate clinician (e.g. Practice Nurse) in line with practice safeguarding policies.

Support the Senior Clinical Care Coordinator for Safeguarding in co-ordinating communication and meetings with third parties e.g Health Visitors, MASH, CCG Safeguarding Leads and Social Workers. This includes taking minutes and updating the patient record in accordance with agreed actions

Update alerts and warnings in the EMIS record according to practice safeguarding protocols when safeguarding concerns are recorded.

Role model for all staff by practicing safeguarding to the highest standard, whilst exercising a high degree of personal and professional autonomy

Care Home Support duties

Provide care coordinator support as required to the Enhanced Healthcare Care Home team with the PCN

Provide coordination and delivery support as required to Care Home MDTs and ward round meetings.

Ensure all meeting actions are recorded and disseminated in a timely manner.

Maintain close working relationships with the EHCH team and Care Home Staff to support care quality improvement which may include supporting joint project initiatives.

Ensure all Care Home safeguarding concerns are registered and appropriately escalated and managed in liaison with the Senior Clinical Care Coordinator for Safeguarding.

Attend EHCH local meetings as required.
