The successful candidate will meet the following :-
- Meet the specific qualification and training requirements as specified in the Nursing Midwifery Standards of proficiency by undertaking the relevant qualification delivered by a Nursing and Midwifery Council (NMC) - approved provider.
- is registered with the NMC and revalidation is undertaken in line with NMC requirements.
Main duties of the job
Key responsibilities in relation to delivering health services:-
- work as part of the practice clinical team to provide and monitor care, under direct or indirect supervision
- improve safety and quality of care at every opportunity
- contribute to the delivery of integrated care
- provide support and supervision to Training Nursing Associates, Healthcare Assistants, Apprentices, and those on learning assignments/placements as required
- perform and record clinical observations such as required
- provide wound care, flu vaccinations, ECGs, and venepuncture, hypertension reviews, long term conditions childhood immunisations and other relevant clinical care as required, in line with the competencies of the role.
- promote health and well-being to all patients.
- care for individuals with complex conditions such as dementia, mental health conditions, and learning disabilities
- advise patients on general healthcare and promote self-management where appropriate, including signposting patients to personalised care colleagues and local community and voluntary sector services
- maintain accurate and contemporaneous patient health records
- enhance own performance through continuous professional development, imparting own knowledge and behaviours to meet the needs of the service

The Much Birch GMS contract was novated to the Federation in February 2024, but the practice team remains unchanged as does the identity of the surgery in the local community.
Much Birch is :-
- A busy rural dispensing practice at the heart of the community.
- Located in the village of Much Birch, between Ross-on-Wye and Hereford city.
- Covering an area reaching from the edges of Hereford to Ross on Wye, and extending into the villages of the Golden Valley.
- List size of circa 5,000+ patients.
A supportive clinical team of GPs, ANPs, Paramedics, Practice Nurses, HCAs, Physiotherapist, Social Prescribers.
