We are delighted to recruit an additional Clinical Pharmacist to join our established PCN Pharmacy Team.

Main duties of the job
As a member of the multi-disciplinary team, you will be expected to provide prescribing advice and support to practice clinicians, staff, and patients. This includes prescription and medication queries, medicines policies and procedures, QOF and local incentive schemes, audit, and medicines safety.
Clinical work at the individual practices will include medication reviews, minor illness consultations and reviews post discharge.
You will have individual supervision from a GP and our Lead Pharmacist and be part of an established team of experienced Pharmacists and Pharmacy Technicians. WBC Primary Care Network includes 3 Hereford city - based practices; Wargrave House Surgery, Belmont Medical Centre and Cantilupe Surgery.
