Job summary
We are excited to recruit
to the post of Health and Wellbeing Coach. This role will work within our Primary
Care Network providing 1:1 personalised support to people who are referred to
our multi-disciplinary Wellbeing Team.
The successful candidate
will have good communication, negotiation and people-management skills and
act with compassion and integrity.
They will have experience
of using coaching approaches, frameworks and models or other helping
strategies, for example, motivational interviewing.
This
is a vital role within the Primary Care Network and the post holder will provide
health and wellbeing coaching for the most vulnerable people in the community,
including the frail / elderly and those with other long-term conditions. They
will work closely with the patient to understand their individual needs and
motivation levels, building a plan and working towards their personalised
identified outcomes.
The successful applicant will be primarily
based at Ledbury Health Partnership and the role will include patient work at
a variety of locations in the community. Some home working may be considered for this
role.
The successful candidate will also support and
may be required to work from other GP Practices based in East Herefordshire Primary
Care Network (PCN) including Nunwell Surgery, Colwall Surgery, and Cradley
Surgery.

Main duties of the job
This is an exciting opportunity to work with GP
practices to help people who need support to develop the knowledge, skills
and confidence to manage their own health and wellbeing.
Specifically the role will support patients with long
term health conditions and/or lifestyle challenges which affect their
wellbeing.
They will work closely with
GP and practice teams to self-manage a caseload, ensuring appropriate support
is available to people, families and carers. The successful candidate will
work alongside social prescribing link workers and care coordinators to
provide an all-encompassing approach to personalised care and will enable
people to work out how best to use the health and care system.
The Health and Wellbeing Coach will work closely
with the practice teams to ensure their role is understood within practices,
reporting back on patients progress to the relevant clinicians and being
part of multi-disciplinary team meetings.
The role will also include ensuring patient activity
and data is accurately recorded on the relevant systems and patient
confidentiality is maintained at all times.

Raise awareness within the PCN of shared decision making and decision support tools and supporting people in shared decision-making conversations.
Work with people with lower activation to understand their level of knowledge, skills, and confidence (their Activation level) when engaging with their health and wellbeing.
Explore and support access to a personal health budget, where appropriate, for their care and support.
Utilise existing IT and MDT channels to screen patients, with an aim to identify those that would benefit from health coaching.
Update patient records and any other systems to monitor patient outcomes and the effectiveness of the service.
