Job summary

The South & West Herefordshire Primary Care Network (PCN) is delighted to be recruiting for a Health & Wellbeing Coach to join our team.

The Health & Wellbeing Coach will partner with other members of the PCN team to support local initiatives, but as an experienced practitioner they will also influence and guide improvement at a county level.

We are committed to having a workforce in which people from diverse backgrounds are supported and empowered to work with local communities to improve health access and outcomes for all and provide culturally appropriate and responsive public services.

*Interviews are expected to be held on Wednesday 18th September 2024*

*This role is for 37.5 hours, five days per week with a preference to include Saturdays*

Main duties of the job

This is an exciting opportunity to work with GP practices to help people who need support to develop the knowledge, skills and confidence to manage their own health and wellbeing.

Specifically the role will support patients, typically across 12-week interventions, as they address key determinants of their health e.g., debt, poor housing, and physical inactivity, as well as other lifestyle issues and low-level mental health concerns. This approach particularly helps people with long term conditions, people who are lonely or isolated, or who have complex social needs which affect their wellbeing.

About us

The South & West Herefordshire PCN GP practices serve rural and market town communities responding to the wide - ranging health needs of the population. The network includes Fownhope Medical Centre, Much Birch Surgery, Pendeen and Alton St practices in Ross on Wye, Kingstone Surgery and Golden Valley Practice in Ewyas Harold. There will be an opportunity to work remotely.

The Health & Wellbeing Coach will be working with established general practice teams who have longstanding relationships with their local community and are committed to providing lifelong care in this beautiful part of Herefordshire.

Job responsibilities

The Health and Wellbeing Coach will work closely with the primary care team to help people who need support to develop the knowledge, skills, and confidence to manage their health and wellbeing.

The Health and Wellbeing Coach will be part of an expanding network team including practice clinicians, Social Prescribers and Mental Health Link Workers offering a whole person approach to address lifestyle issues and encourage proactive prevention of new and existing illnesses.

This multi-disciplinary team approach is an opportunity for network practices to work together to address issues which have a long-term impact on the wellbeing of their patients. The Health and Wellbeing Coach will work closely with colleagues to identify individuals for whom non-medical intervention could improve the quality of their life for example living with a long-term condition/s, chronic pain management, weight management and the impact of adverse life experiences or bereavement.

The Health and Wellbeing Coach will use health coaching skills to support people with lower levels of patient activation to access and utilise community support offers. They will take a non-judgemental approach based on strong communication and negotiation skills that supports personal choice and ensure patients understand the accountability of their own decisions.

Main Responsibilities

Coach and motivate patients through multiple sessions to identify their needs, set goals, and implement a personalised health and care plan.

Provide support to individuals, families, and carers to ensure they are active participants in their own healthcare; empowering them to take more control in managing their own health and wellbeing, to live independently, and improve their health outcomes through:

  • Providing interventions such as self-management education and peer support either in one-to-one sessions of through the use of group consultations.
  • Supporting people to establish and attain goals based on what is important to them.
  • Working with social prescribers and Talk Community teams to connect patients to community-based activities which support their health and wellbeing.

Provide support to local community groups and work with other health, social care, and voluntary sector providers to support the patients health and well-being holistically.

Ensure that fellow PCN staff are made aware of health coaching and social prescribing services and support colleagues to improve their skills and understanding of personalised care, behavioral approaches, and ensuring consistency in the follow up of peoples goals where an MDT is involved.

Raise awareness within the PCN of shared decision making and decision support tools and supporting people in shared decision-making conversations.

Work with people with lower activation to understand their level of knowledge, skills, and confidence (their Activation level) when engaging with their health and wellbeing.

Explore and support access to a personal health budget, where appropriate, for their care and support.

Utilise existing IT and MDT channels to screen patients, with an aim to identify those that would benefit from health coaching.

The role will require meeting with patients and colleagues at a range of bases around the South & West of Herefordshire and occasionally in patients homes. The successful candidate will need to hold a full driving license and have access to a car.

TRAINING REQUIREMENTS

The Personalised Care Institute will set out what training is available and expected for Health and Wellbeing Coaches. This is currently the successful completion of an accredited four-day health coaching training course.

Applications are welcome from those who have already completed the course as well as those who have not. In the event that the necessary accreditation has not yet been acquired, the successful applicant will be supported to undertake the training prior to accepting coaching referrals.

COMMUNICATIONS AND WORKING RELATIONSHIPS

PERSONAL DEVELOPMENT

  • To attend mandatory training and fire lectures regularly and assist with fire policy/evacuation procedure when necessary.
  • To keep up to date with and attend training on revisions to information systems or changes in protocols for the inputting of data in the light of Local and national initiatives. To adhere to agreed protocols at all times.
  • To participate in Individual Performance Review and Personal Development plans on an annual basis and to undertake training and development as identified within these discussions and as indicated by the requirements of the post.

CLINICAL GOVERNANCE AND QUALITY ASSURANCE

  • To realise the importance of confidentiality when dealing with patients and staff, particularly when giving or receiving information over the telephone in accordance with the Data Protection Act.
  • Maintain the high standards of the service by contributing towards individual, team and service objectives and always acting in the best interest of patients.
  • To accurately collect, collate and input data to Information Systems as required by procedures to ensure that patient database records are up to date and accurate.
  • To participate in Individual Performance Review and Personal Development plans on an annual basis and to undertake training and development as identified within these discussions and as indicated by the requirements of the post.
