Job summary
As a Senior PCN Pharmacist, youll play a vital role in supporting the health and wellbeing of our patients across our network of community pharmacies. Working collaboratively within primary care settings, you will lead on medicines optimisation, ensuring patients receive the best outcomes from their treatments. Youll provide expert clinical reviews, oversee prescribing audits, and manage the repeat prescription process in line with best practice and legal standards.
In this role, you will work closely with GPs and other healthcare professionals to deliver safe, effective, and patient-centred care. Youll also be responsible for training and developing healthcare teams, providing health screening and vaccination services, and ensuring full regulatory compliance across all pharmacy operations. Additionally, you will contribute to service delivery by monitoring performance, supporting resource planning, and driving continuous improvement in line with organisational and contractual goals.
Main duties of the job
As a Senior PCN Pharmacist your duties include, but are not limited to:
- Prioritising patient care, seeing patients with one or multiple medical problems where medicine optimism is required, and undertaking clinical medical reviews for patients, ensuring patients receive the best use from their prescriptions.
- Working closely with GPs, and other practicing heath care professionals where appropriate, answering any patient medicine questions they have.
- Undertaking clinical audits of prescribing in areas directed by the GPs, communicating results and implementing changes.
- Managing the repeat prescribing reauthorisation process by clinical review of patient requests in line with review dates, and flagging those needing review.
- Training and developing the primary healthcare teams on therapeutics and medicines optimism.
- Ensuring legal compliance within the pharmacy, reviewing labels, doses, and securing inventories-controlled drugs in line with regulations.
- Offering health-screening including blood-pressure monitoring, diabetes screening flu and other vaccine administration.
- Providing regular updates and monitoring of KPIs to the operational management team.
- Resource planning to meet service and contractual requirements.

- Reviewing the on-going need for each medicine, a review of monitoring needs and an opportunity to support patients with their medicines ensuring they are getting best use from their prescriptions
