o As a Young Persons Practitioner to provide a county -wide holistic support service for 10- to 25-year-olds with needs related to substance misuse, including needs that arise because of familial substance misuse.o The engagement of vulnerable young people in a variety of interventions, using a young person-centred approach, empowering them to reduce risk, increase resilience and achieve positive outcomes.o To manage a caseload of young people from a mixture of community and multi-agency referral pathways including health, social care, education, and criminal justice.o Working flexibly across the community and within our multi-agency partnership settings on an outreach basis to meet the needs of young people and their families.o Facilitating drop-in and group work sessions to support engagement with harder-to-reach young people.o Showing resilience and taking a solution focused approach to crisis managemento Work to promote recovery and positive outcomes for young people and families.

Main duties of the job

o To promote a strength- based approach with young people to empower change, to reduce risk and build resilience.o To support and build relationships with 10- to 25-year-olds, and their families, who have a variety of needs related to substance misuse, either their own substance use or somebody else's.o Hold a caseload of young people and complete all recording and case management administration for your caseload accurately and promptly, adhering to GDPR and Data protection policies.o Develop co-produced, outcome focused support plans that reflect each participant's personal strengths and goals. Support plans will be holistic, covering all aspects of the young person's wellbeing. This will include the engagement of parent/carers, and family members where appropriate.o To work as part of a multi-agency team delivering interventions to safeguard children and vulnerable adults at risk and to reduce drug and alcohol related harm to young people, families, and communities.o To provide information and advice to colleagues regarding the risk and protective factors surrounding young people with complex needs.

Qualifications

Essential

  • Foundation Degree / HNC in related field which may include counselling, social work, nursing, psychology, human resources, social studies or equivalent experience

Experience

  • Demonstrate experience and understanding of working with young people with complex needs
  • Experience of completing risk assessments
  • Excellent communication skills and a willingness to promote service user involvement
  • Case management experience & the ability to work under pressure
  • Experience of working within multidisciplinary teams with a proven ability to work collaboratively across professional boundaries
  • Experience of working with techniques such as CBT, Motivational interviewing, and Solution focused therapy

Skills and Knowledge

  • Knowledge of drugs including alcohol and their effects
