To work within the service users and multi-agency team members in an essential liaison role to draw in local community assets to support the delivery of Recovery focussed outcomes, promoting and creating opportunities in visible recovery, harm reduction and brief interventions in the hub reception areas.

Main duties of the job

o Work as an integral part of the service in delivering high quality interventions within the hub. o Work with and support the Inclusion multi-disciplinary team o Facilitate harm reduction interventionso Supporting service users who are attending the reception area of the service.o Contributing to service users working collaboratively to build their recovery capital and ensuring positive recovery outcomes.o Ensuring service users have scheduled appointments and remove barriers to attendance.o Supervising recovery supporter volunteers.o Promoting visible recovery in the hub, especially in reception areas. o Reducing drug and alcohol related harm to service users and the wider communityo Ensuring that your work is evidence based, up to date and in accordance with current policy and legislation. o Ensuring the reception environment is welcoming and accessible.

  • Provide harm reduction advice, information, and support with an emphasis on safer drug use, safer sex, HIV, Hepatitis A, B and C and other blood borne infections
  • Be responsible for maintaining a full case load of complex clients as the key worker.
  • To make home visits consistent with need.
  • To carry out comprehensive assessments, construct care packages with multi-disciplinary colleagues where required, review care plans in a structured format and manage onward referral.
  • To assist and support medical and nurse prescribers during harm reduction and prescribing clinics.
  • To provide a range of structured psycho-social interventions including CBT based brief solution focused therapy and motivational enhancement techniques.
  • Make domiciliary visits to service users particularly for those with disabilities and to those with family responsibilities.

Essential

  • Foundation Degree / HNC in related field which may include counselling, social work, nursing, psychology, human resources, social studies or equivalent experience

Experience

  • Considerable experience of working with drug users
  • Experience of working with techniques such as CBT, Motivational interviewing, and Solution focused therapy
  • A commitment to and understanding of harm reduction & recovery approaches to working with drug users.
