We have an exciting opportunity for a passionate and caring individual to join our dynamic team. The postholder will be part of the Wessex Clinical Van service, providing outreach treatment pathways for viral Hepatitis across Hampshire, Dorset and the Isle of Wight. This will include health screening and assessment, other health and wellbeing treatment, signposting and onward referral to the relevant services. Our service users are at risk of have having life limiting/life restricting co-morbidities and are at risk of earlier death. This service will aim to increase access to Hepatitis C treatment pathways and other Health and Wellbeing services and play a significant role in identifying illness early with the aim of accessing the right treatment at the right time
Main duties of the job
This post is for a 12 month fixed term contract. As part of the Wessex Clinical Van team you'll be an essential part of our care approach, working collaboratively across Wessex with multidisciplinary partners.

- To provide a clinical service to people accessing the Wessex Clinical Van that is consistent to their needs including individual treatment and care packages underpinned by a sound evidence base.- To deliver an effective health and wellbeing screening and assessment that meets the individual needs of each service user. This will include developing relationships that allow for other Health Services to work in partnership.- To develop and maintain effective liaison with other agencies including Criminal Justice Teams, non-statutory services, Specialist Drug and Alcohol Services, primary care, shared care GPs, Community Mental Health Teams, IAPT Service, Social Work Teams, Sexual Health Services, Hospitals (including Specialist Healthcare departments).- To support and assist in development of service protocols and policies in line with national guidance for own specialist area.- To lead on the Return to Care Project across Wessex, working closely with the Coordinators to find, engage and treat people who are not currently engaged in care.- Promote partnership work and develop integrated care pathways with local health and wellbeing agencies, both in primary and secondary care
