  • Able to support people to progress through their sentence and onto resettlement in the community.

Main duties of the job

You will deliver a variety of specialist communication assessments, interventions and advice in the clinical area of complex social interaction and communication difficulties. You will be responsible for the assessment, planning, implementation and evaluation of evidenced based interventions. The post holder will manage a caseload of patients, practicing within the framework of the multi-disciplinary team and Secure Stairs methodology and will provide a care coordinator role and communicate effectively with other professionals to ensure through care support where appropriate. You will be a reflective practitioner, eager to work collaboratively and contribute to the delivery of an effective, dynamic, specialist service. You will work with individuals from initial assessment through to discharge, which can include transferring care to other providers such as hospitals or CMHTs whilst working with a variety of agencies in the Offender Health Pathway. You will help to rehabilitate and support people to live decent and purposeful lives as members of the community. You will have a strong engagement and motivation skills and be able to work well with service users with complex needs which can include health needs of learning disability, forensic/offending history and mental health needs.
