Main duties of the job
Working effectively within the team you will provide evidence based interventions. You will manage a caseload and will be required to undertake assessments and reviews. Using the Care Programme Approach, you will be a care coordinator, communicating effectively with those involved in the care of people in custody up to release. The role will require you to be a reflective practitioner, eager to work collaboratively with individuals, carers and other professionals to contribute to the delivery of an effective, dynamic, integrated mental health and substance misuse service. You will work with individuals from initial assessment through to discharge, which can include transferring care to other providers such as hospitals or CMHTs whilst working with a variety of agencies in the Offender Health Pathway. You will help to rehabilitate and support service users to live hopeful and fulfilling lives as members of the community.
