Job summary

Are you passionate about empowering individuals to take control of their health and wellbeing? Battersea PCN Ltd is seeking a dynamic and compassionate Health and Wellbeing Coach to join our team for a 12-month maternity cover contract. This is a fantastic opportunity to be part of an innovative and supportive environment, working across five GP practices within the Battersea Primary Care Network.

As a Health and Wellbeing Coach, you will play a vital role in supporting individuals particularly those with long-term health conditions such as diabetes, obesity, or at risk of cardiovascular disease to improve their health outcomes. Through one-to-one and group coaching, you'll help clients build the knowledge, confidence, and skills to make positive lifestyle changes.

You'll be instrumental in shaping future coaching services, collaborating with social prescribers, assistant psychologists, care coordinators, GPs, and nurses. You'll also contribute to exciting projects such as our Youth Clinic, menopause support programme, and Community Health Worker initiative.

Main duties of the job

  • Deliver personalised, evidence-based health coaching sessions (individual and group)
  • Use motivational interviewing and behaviour change techniques
  • Empower clients to set achievable health goals and action plans
  • Collaborate with a multidisciplinary team across the PCN
  • Signpost to relevant community and wellbeing services
  • Collect and evaluate data to support service improvement
  • Promote awareness of the benefits of health coaching among staff and stakeholders

Battersea PCN comprises of 5 GP surgeries in a compact area of North Battersea (Bridge Lane Group Practice, Lavender Hill Group Practice, Battersea Fields Practice, Queenstown Road Medical Practice, Battersea Rise Group Practice) with a combined population of approximately 57,000 patients.

We enjoy excellent collaborative working relationships and are committed to developing the work of the Health Coaches across the 5 practices. Our Lead Health and Wellbeing Coaches offer support for existing services and innovation, particularly around group and 1-2-1 patient consultations.

  • Empower people to take control of their own wellbeing by building their confidence, motivating them to set goals and creating personalised action plans.
  • Support people to overcome their personal barriers to make positive lifestyle changes.
  • Provide ongoing motivational support and follow up to ensure long term behaviour changes.
  • Support people to access appropriate community support and self management education.
  • Treat everyone with dignity, and respect different cultures, religions, ways of life and other personal characteristics at all times.

Communication and Collaboration

  • To work alongside the Lead HWBC to develop services including individual and group coaching, working closely with lead GPs and other members of our Adult and Young Peoples personalized care teams.
  • To work collaboratively with the PCN Management team, the Lead HWB Coach and practice staff to ensure the provision and ongoing development of the coaching service
  • Work alongside primary care professionals in the practice and PCN teams to provide joined-up personalised care
  • Build excellent relationships with primary care teams and other key stakeholders.
  • Build knowledge of community support and develop partnerships to ensure signposting is safe and effective.
  • Act as a champion of personalised care and health coaching, increasing awareness and promoting its benefits to primary care staff and other audiences.
  • Develop a peer network, building relationships with other health coaches at local and national level, in order to gain insights into best practice and innovation in health coaching in other areas of London and the UK.

Service Quality and Development

  • Collect and record outcome, monitoring and impact data using required systems, contributing to the evaluation of the service.
  • Manage own case load to ensure clients receive appropriate levels of support and expectations are met.
  • Gather client feedback to evaluate the quality of the service and to inform service changes and developments.
  • Keep up to date with key changes in the health and social care sector in order to shape and develop the service, including those relating to good practice and policy.
  • Lead by example by demonstrating excellent customer service, delivery skills, organisation and professionalism.
  • Work with the Lead Coach and PCN Clinical Directors to identify your own support needs and undertake continual personal and professional development.
