Job summary
We are currently seeking a dedicated and experienced Practice Nurse to join our busy Primary Care Network working across 6 practices.
This is a new role for the PCN - working in a mix of a hub and practice based model.
As a Practice Nurse, you will be responsible for providing high-quality nursing care to our patients and working/supporting with the ARRS Roles in the PCN as well as the wider practice teams.
You will be supported by our Enhanced Nurse and ARRS GP.
Main duties of the job
Previous PCN employment is not essential, but experience in a general practice or similar setting is required. Familiarity with PCN ways of working is a strong advantage.
- Conducting health assessments, including physical examinations and health screening
- Providing health education and advice to patients on a range of topics, such as healthy living and disease prevention
- Administering vaccinations and other injections
- Taking blood samples and carrying out other diagnostic tests
- Supporting patients with long-term conditions, such as diabetes and asthma
- Supporting the practice with the management of chronic diseases
- Cervical Smears
Requirements:
- Registered Nurse with a valid Nursing and Midwifery Council (NMC) pin
- Minimum of 2 years' experience working as a Practice Nurse
- Experience in chronic disease management
- Experience in Cervical Smears
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
