Advanced Clinical Practitioner (ACP)

Job summary

We are seeking a Primary Care Advanced Clinical Practitioner (ACP) to join our dynamic team.

This role is suitable for both experienced ACPs and those who have recently qualified. The successful candidate will provide high-quality, patient-centred care, working autonomously within their scope of practice to assess, diagnose, treat, and manage a wide range of conditions.

For ACPs, we welcome applications from both experienced practitioners and those newly qualified, with support available to help develop confidence and competence in the role.

Main duties of the job

Key Responsibilities:

  • Conduct comprehensive patient assessments, taking medical histories and performing clinical examinations.

  • Diagnose, manage, and treat acute and long-term conditions, including prescribing where qualified.

  • Work autonomously while collaborating with GPs, nurses, and other healthcare professionals.

  • Provide patient education, health promotion, and preventative care.

  • Undertake clinical audits and contribute to service improvement initiatives.

  • Maintain accurate and timely documentation in line with professional and legal requirements.

  • Participate in training and development programmes to enhance clinical skills.

About us

Meir PCN is a membership comprising of six practices - Dr Raju & Partners, Adderley Green Surgery, Meir Medical Practice, Meir Park & Weston Coyney Medical Practice, Trinity Medical Practice and Willow Bank Surgery - covering the Meir, Longton & Blythe Bridge areas of Stoke on Trent.

We currently have a PCN population of approximately 37,500 patients and the PCN staff work in conjunction with the local general practices, local services and other organisations to support both local and national priorities.

We take immense pride in how our PCN is shaping and how we are providing excellent support and care to our practice colleagues and patients.

Our goal is to improve health & care locally and to meet everyday health and care needs for patients by connecting primary care systems and using creative thinking to develop, improve and support great local services.

Our dedicated multidisciplinary team includes GPs, Advanced Clinical Practitioners, Practice Nurses, Pharmacists, and other healthcare professionals working collaboratively to provide comprehensive and accessible care.
