An exciting opportunity has arisen for a Registered Nurse to join our Hospice
at Home team to provide care and support to patients with terminal
illnesses, whilst also offering vital emotional support for their families
As a registered nurse you would
cover a combination of night shifts either by providing one to one care in
patients homes or work on the Roaming Service which offers more flexible care
and symptom management to patients referred to the Hospice at Home service.
In return, you will have the opportunity to access a generous portfolio
of training and education that will enable you to comply with NMC Revalidation.
You will be able to access management support, clinical supervision, and an
occupational pension scheme (NHS Pension Scheme rights are preserved).
Main duties of the job

Ensuring we have the skills and experience to deliver and promote
excellence in end-of-life care provision.
Working in partnership with other local end of life service providers to
ensure the best possible patient experience is achieved.
Developing services to reflect the changing needs of the diverse
community we serve.
Our departmental objectives are to provide a nursing service,
which is available 7 nights a week, for people with a limited life expectancy,
to enable them to remain at home if that is their wish.
To provide high quality palliative care and support in conjunction
with the Primary Health Care Teams, offering skilled nursing care to those in the
terminal phase of their illness, or with chronic incurable conditions.
To support and encourage the sharing knowledge
and skills withing the team.

- 2 years minimum post registration experience
- Experience of providing palliative care.
- Up to date evidence-based knowledge of current end of life care

- Experience of providing community palliative care.
- Experience in bereavement support

- Own car and driving licence and be able to drive a manual and automatic car.
