Job summary
As a Health Care Assistant for
Treetops Hospice at Home, you will deliver care and support to people with
terminal illnesses in the last few months and weeks of life. You will carry out
a range of care duties including helping patients meet their hygiene needs,
monitoring symptoms and keeping detailed records and emotional support to the
patient and their families.
Our approach is predominantly one
to one, so you will often be working independently using your own initiative in
line with patient need and Registered Nurse support.
Main duties of the job
We are looking for
individuals with exceptional caring and interpersonal skills with an empathic
and compassionate nature to join our Hospice at Home team. Your key objective
will be to provide individualised palliative care to patients as part of the
Hospice at Home night service, either through the provision of one-to-one night
support in a patients home or as part of a roaming team of two covering the
Southern Derbyshire geographical area. You will provide respite, help and
support to the families and carers of individual patients. Give skilled nursing
care within the care plan of the District Nurse, and in accordance with the
medical care prescribed by the GP.
Help/prompt the
patient to self-medicate if it is part of their nursing care plan or to assist
patient with medication administration once Level 1 Support for the Patient
with Prescribed Medication course is completed.
Demonstrate simple nursing
procedures, where appropriate, to carers or colleagues to assist in promoting
and maintaining patient comfort.
