Job summary

You will carry out a range of care duties including helping patients meet their hygiene needs, monitoring symptoms and keeping detailed records and emotional support to the patient and their families.

Your key objective will be to provide individualised palliative care to patients as part of the Hospice at Home night service, either through the provision of one-to-one night support in a patients home or as part of a roaming team of two covering the Southern Derbyshire geographical area. You will provide respite, help and support to the families and carers of individual patients. Give skilled nursing care within the care plan of the District Nurse, and in accordance with the medical care prescribed by the GP.

Help/prompt the patient to self-medicate if it is part of their nursing care plan or to assist patient with medication administration once Level 1 Support for the Patient with Prescribed Medication course is completed.

Demonstrate simple nursing procedures, where appropriate, to carers or colleagues to assist in promoting and maintaining patient comfort.

Our vision is that everyone living within the communities we serve has access to end-of-life care of the highest quality and we need you, our best, most valued asset, to deliver our mission of making every day count through giving the highest quality support and care to our patients and carers.

In return you will have access to a comprehensive training and education programme to help develop your skills and knowledge. Treetops Hospice is an amazing organisation to work for and your health and wellbeing is important to us. Our employees are passionate, caring people who believe in and are driven by our vision and values.
