Job Overview

Bradford City Primary Care Network 4 is looking for a Lead Social Prescriber that can work with PCN management to develop the existing Social Prescribing service and team. As well as working with the existing Social Prescribing team the Lead Social Prescriber will also work to support and promote our Mental Health Service working alongside the Mental Health Practitioner and experienced Counsellors.

The Social Prescriber supports taking a holistic approach to an individuals health and wellbeing. The role will involve connecting people to community groups and statutory services for practical and emotional support.

The Social Prescriber will provide support for low to medium intensity patients with mental and social health conditions to overcome barriers. The Social Prescriber will be embedded within Primary Care Network multi-disciplinary teams to provide personalised support to individuals, to enable them to take control of their wellbeing and improve their health outcomes.

The successful candidates will have excellent communication skills and a high level of IT literacy.

  • Assess how far a patients health and wellbeing needs can be met by services and other opportunities available in the community
  • Co-produce a simple personalised care and support plan to address the patients health and wellbeing needs by introducing or reconnecting people to community groups and statutory services, signposting where appropriate
  • Evaluate how far the actions in the care and support plan are meeting the patients health and wellbeing needs
