Job summary
The successful candidate will work across the 5 GP Practices in the Primary Care Network,
supporting (or leading) projects ranging from medicines reconciliation
post-discharge, medicine safety alert processes, daily medicine-related tasks,
involvement in seasonal vaccination programmes and facilitating medication
reviews.
We expect contribution to safe and effective prescribing across the PCN,
acting on best practice from local and national guidelines.
Overarching
goals are to help keep patients safe and well out of hospital and reduce the
demands on general practice regards medicine use.
The
post holder will work within their clinical competencies as part of a
multi-disciplinary team to provide expertise in clinical medicines management,
provide face to face structured medication reviews, manage long term
conditions, management of medicines on transfer of care and systems for safer
prescribing, manage repeat prescription authorisations processes, while
addressing both the public health and social care needs of patients in the GP
practices that make up the PCN.
The post holder will develop and encourage a positive culture of
medicines optimisation principles to assist the PCN in achieving local and
national medicines-related objectives.
Main duties of the job
Promotes high quality cost-effective & evidence-based use of
medicines within the PCN.
Be a contributing team member to the PCN Pharmacy Team to respond to
medication-related queries generated by patients, GPs, other health care
professionals and staff.
Assist the PCN Pharmacy Team to develop and ensure a safe
efficient medicines optimisation programme including repeat prescribing,
medication review & medicines reconciliation services.
Be proficient with TPP/SystmOne GP prescribing system & supporting IT to record personally generated information & maintain a
database of information relating to the work done in the practices.
Identify & produce recommendations for medication
optimisation for clinical pharmacists, GPs & other multi-disciplinary
team members.
Work with care home staff to improve safety of medicines ordering & administration if required.
Encourage good management of common/minor/self-limiting ailments,
including promotion of self-care & homely remedies.
Liaise with all members of the local PCN including community
pharmacies & community nursing team, secondary care providers & regional
CCG teams on prescribing-related matters.
Aim for good collaborative working practices & develop
relationships with other professionals across PCNs & the wider health & social care system.
