Small but keen team needs another Pharmacy Technician!

As a Pharmacy Technician, you will be able to enrol on the CPPE Primary care pharmacy education pathway and gain the essential training needed to work in various patient-facing roles in primary care networks as part of a multidisciplinary team.
The roles take advantage of the Additional Roles Reimbursement Scheme; suitable candidates must meet the criteria for funding.
You will be expected to use your professional skills to carry out a variety of tasks including medication reviews, medication reconciliation, and reducing inappropriate prescribing in risk-stratified patients.
We expect commitment and expertise to advise on medicines optimisation, safety and appropriate prescribing in line with local and national policies.
Main duties of the job
Provide specialist expertise in the use of medicines
Support PCN staff in the delivery of services to people in their own homes or local care homes
Undertake face to face and telephone-based patient consultations, reviewing patients with potential medication issues and long term conditions
Develop and run medicines management work to maximise safe, cost-effective and patient-centred best practice in prescribing
Support patients to take their medications to get the best from them, reduce waste and promote self care
Monitor and improve prescribing and adherence to local and national prescribing policies through audit and searches

CCB PCN provides proactive and coordinated care across a local
population of 58,000 people. We are a dynamic, friendly and supportive PCN with
much experience in training GPs and other healthcare professionals. The PCN
promotes multi-disciplinarian working by hiring Social Prescribers, Physician
Associates, Advanced Nurse Practitioners, and Paramedics.
Situated close to junction 17 M4, we have employees successfully
commuting from Bristol to the west and Swindon to the east. There is free parking on site or a short walk

Pharmacy teams in primary care are becoming well-established in order to keep patients safe, out of hospital and reduce the demands on general practice regards medicine use.
Pharmacy Technicians play an important role, complimenting clinical pharmacists, community pharmacists and other members of the PCN multi-disciplinary team. While Pharmacy Technicians do not prescribe nor make clinical decisions, they do work under supervision to ensure the effective and efficient use of medicines. In the PCN, their core responsibilities cover clinical, technical, and administrative roles.
The purpose of the role is to maximise safe, cost-effective, prescribing to improve the quality of patient care. The post-holder will help patients get the best from their medicines, use existing protocols to maximise cost-effective preparations, minimising clinical risk and reducing avoidable wasted medicines. As well, the post-holder will develop and encourage a positive culture of medicines optimisation principles to assist the PCN in achieving local and national medicines-related objectives.

To help achieve efficiencies and effectiveness within the prescribing activities of the PCN group of GP practices
To promote high quality cost-effective and evidence-based use of medicines within the PCN
To work within the Pharmacy Team, with clinical pharmacists to respond to medication-related queries generated by patients, GPs, other health care professionals and staff
To work within the Pharmacy Team, with clinical pharmacists to develop and ensure a safe efficient medicines optimisation programme including repeat prescribing, medication review and medicines reconciliation services
