Job summary

Small but keen team needs another Primary Care Pharmacist!

If you are enthusiastic, patient-focussed, and independently motivated, apply to join the CCB PCN.

You will be able to enrol on the CPPE Primary care pharmacy education pathway and gain the essential training needed to work in various patient-facing roles in primary care networks as part of a multidisciplinary team.

The roles take advantage of the Additional Roles Reimbursement Scheme; suitable candidates must meet the criteria for funding.

You will be expected to use your professional skills to carry out a variety of tasks including medication reviews, medication reconciliation, and reducing inappropriate prescribing in risk-stratified patients.

We expect commitment and expertise to advise on medicines optimisation, safety and appropriate prescribing in line with local and national policies.

Main duties of the job

  • Provide specialist expertise in the use of medicines
  • Support PCN staff in the delivery of services to people in their own homes or local care homes
  • Undertake face to face and telephone-based patient consultations, reviewing patients with potential medication issues and long term conditions
  • Develop and run medicines management work to maximise safe, cost-effective and patient-centred best practice in prescribing
  • Support patients to take their medications to get the best from them, reduce waste and promote self care
  • Monitor and improve prescribing and adherence to local and national prescribing policies through audit and searches

CCB PCN provides proactive and coordinated care across a local population of 58,000 people. We are a dynamic, friendly and supportive PCN with much experience in training GPs and other healthcare professionals. The PCN promotes multi-disciplinarian working by hiring Social Prescribers, Physician Associates, Advanced Nurse Practitioners, and Paramedics.

Situated close to junction 17 M4, we have employees successfully commuting from Bristol to the west and Swindon to the east. There is free parking on site or a short walk from Chippenham train station.

Job responsibilities

Clinical Pharmacy in primary care is becoming well-established in order to keep patients safe, out of hospital and reduce the demands on general practice regards medicine use.

The post holder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines management, provide face to face structured medication reviews, management of medicines on transfer of care and systems for safer prescribing while addressing both the public health and social care needs of patients in the GP practices that make up the PCN.

The post holder will provide leadership on quality improvement and clinical audit and well as managing some aspects of the Quality and Outcomes Framework. This role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver an excellent service within general practice.

As well, the post holder will develop and encourage a positive culture of medicines optimisation principles to assist the PCN in achieving local and national medicines-related objectives.

Main Duties

Help achieve efficiencies and effectiveness within the prescribing activities of the PCN group of GP practices

Promote high quality cost-effective and evidence-based use of medicines within the PCN

Work within the Pharmacy Team, with other clinical pharmacists to respond to medication-related queries generated by patients, GPs, other health care professionals and staff

Work within the Pharmacy Team, with other clinical pharmacists to develop and ensure a safe efficient medicines optimisation programme including repeat prescribing, medication review and medicines reconciliation services

Become proficient with TPP/SystmOne GP prescribing system and supporting IT to record personally generated information and maintain a database of information relating to the work done in the practices

Undertake clinical medication reviews either face to face or via telephone support on identified stratified-risk patients that may include residents in care homes, in their own, with long-term conditions, and/or learning disabilities

Identify and produce recommendations for medication optimisation for senior clinical pharmacists, GPs and other multi-disciplinary team members

Work with care home staff to improve safety of medicines ordering and administration if required

Work to encourage good management of common/minor/self-limiting ailments, including promotion of self-care and homely remedies

Contribute to the safety of drug monitoring processes and drug alerts

Analyse, interpret and present medicines data to highlight issues and risks to support decision making

Advocate and implement local, regional and national formulary recommendations, including public health campaigns

Liaise with all members of the local PCN including community pharmacies and community nursing team, secondary care providers and regional CCG teams on prescribing-related matters

Aim for good collaborative working practices and develop relationships with other professionals across PCNs and the wider health and social care system

Work with your line manager to undertake continual personal and professional development, taking an active part in reviewing and developing the role and responsibilities.

Adhere to organisational policies and procedures, including confidentiality, safeguarding, lone working, information governance, and health and safety.
