Are you a friendly, organised, and compassionate professional who thrives on supporting patients and helping services run smoothly? DHU Healthcare is seeking aCare Navigatorto join our Primary Care teams, providing essential front-line support to patients and clinicians across our Derbyshire practices. The ideal candidate will be available to work between 08:00 and 18:30, Monday to Friday, with their regular contracted hours agreed in advance. On occasion, additional cover may be required, and we will always provide as much notice as possible.
The main site is in the village of Eyam, in the heart of the Peak District with the branch site located in nearby Bradwell

Main duties of the job
Acting as the first point of contact for patients, providing a warm and professional welcome
Navigating patients to the right care quickly and efficiently whether thats a GP, nurse, pharmacist, or other services
Managing appointment bookings and practice communications with accuracy and care
Supporting clinical teams by handling essential admin and digital systems
Providing clear information and signposting to local health and community services
Contributing to practice efficiency, patient satisfaction, and quality improvement initiatives
Working collaboratively with GPs, nurses, and other healthcare professionals
