We are looking for an enthusiastic and patient focused Care Navigator to join our friendly Primary Care team.

As the first point of contact for our patients, you will play a key role in ensuring they receive the right care, at the right time, from the right professional. This is an important and rewarding role within the practice, suited to someone who thrives in a busy environment and is committed to delivering excellent customer service.

The role will involve working across both Eyam and Bradwell Surgery as required to support service delivery.

As a Care Navigator, you will

  • Act as the first point of contact for patients and visitors, providing a welcoming and professional service both face to face and on the telephone.
  • Book appointments and manage patient enquiries using the clinical system, ensuring patients are directed to the most appropriate clinician or service.
  • Signpost patients to alternative services such as community pharmacy, social prescribing, physiotherapy and other community-based support where appropriate.
  • Process repeats prescription requests, communicate test results and ensure messages are passed accurately and promptly to the relevant team member.
  • Support the smooth daily running of the practice, including monitoring emails, handling post, maintaining reception and waiting areas, receiving payments and opening closing the premises as required.
  • Adhere to all practice policies including confidentiality, safeguarding and Health and Safety, while providing flexible support across both sites to meet service needs.
