The post holder will be required to work at St Peter's Hospital, Guildford Road, Chertsey, Surrey, KT16 0PZ, and at Weybridge Health Centre, 22 Church Street, Weybridge, Surrey, KT13 8DW

post holder will work as part of our team to provide high-quality care to the
patients of our 37 practices across our Federation. The Health Care Assistant
will work autonomously to deliver safe and effective nursing care. Previous
primary care experience is essential.

Job Summary
The post holder will work as part of our team to provide high-quality care to the patients of our 37 practices across our Federation. The Health Care Assistant will work autonomously to deliver safe and effective nursing care. Previous primary care experience is essential.
Key Responsibilities
- Phlebotomy
- Wound care and dressings
- NHS Health Checks and other preventive health activity
- Chronic disease monitoring and reviews within scope of competence
Clinical Duties
- Undertake venepuncture and ensure high standards of practice across the Federations phlebotomy service.
- Deliver wound assessment and management, including dressing changes and post-operative care. Including removal of clips and sutures
- Conduct NHS Health Checks and contribute to proactive health promotion initiatives.
- Undertake chronic disease reviews (e.g., hypertension, diabetes, COPD, asthma) in line with protocols and within scope of competence.
- Accurately record clinical data on the relevant clinical systems (EMIS).
- Escalate concerns or clinical issues appropriately to the supervising Lead Nurse or GP.
- Recognise deterioration in wounds and refer onto the complex wound care clinic as necessary
