Fixed term 12 months contract, working flexible18.75 hours per week (0.5 WTE) per postholder ( 2 posts available)

The postholder(s), being hosted by NICS, will work closely with the Respiratory Clinical Leads at Surry Heartlands ICB to support improving identification and review of patients with asthma. The postholder(s) will deliver a quality highly specialised improvement programme, focusing on either West Surrey or East Surrey GP facing practices. The post holder will proactively support GP practices, by helping them to identify and manage uncontrolled asthma patients, aged 18 and over to embed evidence-based guideline care to improve asthma management across the entire asthma spectrum - from mild patients who may be on Short acting beta 2 agonists (SABA) monotherapy alone, right through to severe asthma patients who may be on long term oral steroids and high dose Inhaled corticosteroids (ICS). The post holder will conduct face to face patient reviews and educate both practice staff and staff within the locality in asthma management.

Job responsibilities

The postholder(s) working closely with the Respiratory Clinical Leads at the ICB will support Surrey Heartlands ICB to improve identification and review of patients with asthma. The postholder(s) will deliver a quality highly specialised improvement programme, focusing on either West Surrey or East Surrey GP facing practices. The post holder will proactively support GP practices, by helping them to identify and manage uncontrolled asthma patients, aged 18 and over to embed evidence-based guideline care to improve asthma management across the entire asthma spectrum - from mild patients who may be on Short acting beta 2 agonists (SABA) monotherapy alone, right through to severe asthma patients who may be on long term oral steroids and high dose Inhaled corticosteroids (ICS). The post holder will conduct face to face patient reviews and educate both practice staff and staff within the locality in asthma management.

The postholder(s) will support the CORE20Plus5 delivery for patients with Long Term Conditions (respiratory) by:

Providing additional capacity to support proactive patient reviews, alleviating pressure from the system ensuring care aligns with NICE and local guidelines

Act as an independent non-medical prescriber and contribute to adherence of medi-cines optimisation strategies

Deliver a positive reduction in environmental burden from carbon emissions from re-duced use of metered dose inhaler SABA therapy, and increased use of dry powder inhalers for AIR and MART.

Promote patient self-management and deliver individualised care plans

Encourage uptake of Influenza, Pneumococcal and Covid Vaccinations where they meet eligibility criteria.

Improve the equity in standard of care for Asthma patients.

Reducing the clinical burden in primary and secondary care from increasing appropri-ateness of referrals and reducing unscheduled visits for asthma care (hospital admis-sions from severe exacerbations and GP visits for exacerbations).

Provide support for education to GP practices as needs are identified to help improve HCP knowledge and capabilities regarding asthma care

Provide educational support for the rollout of SENTINEL to GP Practices

Providing a sustainable impact on the improved capability of HCPs to manage asthma on project completion e.g. by role modelling clinics.

Ensuring effective communication and collaboration across multidisciplinary teams and organisational boundaries.

Build relationships with all stakeholders to enable delivery of identified programmes and targets which support integration and continuity of care for patients with respiratory conditions.

Key Responsibilities

To lead and promote high quality specialist asthma care within Primary Care

Clinical

Review a caseload of patients with respiratory needs, providing evidence-based as-sessment, planning, implementation, and evaluation of care with Primary Care.

Liaise and support Community Services and Secondary care to support patient care and improve overall outcomes.

Undertake detailed clinical assessments, including diagnostic investigations (e.g., spi-rometry, FENO, peak flow, airway reversibility).

Develop and review individualised self-management plans and support patients in life-style changes.

Provide specialist advice to patients, carers, and professionals, including education on inhaler technique and medication adherence.

Participate in Practice MDT meetings as appropriate.

Ensure accurate and timely documentation in line with local and national standards.

Service Development & Management

Lead and promote integration of respiratory care between primary, secondary, and community services.

Identifying patients for review, and support GP practices to set up systems and pro-cesses for improving ongoing care of asthma patients

Develop and implement policies, protocols, and guidelines in line with national and lo-cal standards.

Monitor and report on service outcomes, including cost savings and quality improve-ments.

Participate in and lead service audits or evaluations for the project.

Attend and participate in ICB respiratory Network meeting relating to this project
