About the Role
We areseeking a motivated and compassionateHome Visiting Practitionerto join our primary care team. The successful candidate will providesame-day home visitsto patients who are housebound or too unwell to attend the surgery.
Working closely with ourGPs, nurses, and wider multidisciplinary team, you will assess patients in their homes, provide clinical management where appropriate, and help ensure patients receive the right care at the right time.
This role plays an important part in supporting patient care while improving GP capacity within the practice.

Conducthome visitsfor patients who are unable to attend the surgery.
Undertakeclinical assessment, examination, and managementof patients in their homes.
Develop and implementmanagement plans, including prescribing where qualified.
Providesafety-netting adviceand arrange follow-up care where required.
Liaise withGPs, district nurses, community teams, and secondary careservices.
Maintain accurate and timelyclinical recordsusing the practice clinical system.
Identify patients requiringurgent escalation or hospital admission.

We are one of the largest GP practices in Wales serving over 25,500 patients across three sites. We have a great team of 7 partners, 8 salaried GPs, 5 nurses, 4 HCAs and a clinical pharmacist. We have first contact physio every day.
