About the Role

We areseeking a motivated and compassionateHome Visiting Practitionerto join our primary care team. The successful candidate will providesame-day home visitsto patients who are housebound or too unwell to attend the surgery.

Working closely with ourGPs, nurses, and wider multidisciplinary team, you will assess patients in their homes, provide clinical management where appropriate, and help ensure patients receive the right care at the right time.

This role plays an important part in supporting patient care while improving GP capacity within the practice.

We are one of the largest GP practices in Wales serving over 25,500 patients across three sites. We have a great team of 7 partners, 8 salaried GPs, 5 nurses, 4 HCAs and a clinical pharmacist. We have first contact physio every day.

  • 5 Practice Nurses (including an Independent Prescriber)

  • GP Trainees (GPSTs) and Foundation Doctors

  • HCA-led Chronic Disease Management Clinics

  • Daily direct access physiotherapy

  • Advanced workflow with trained admin staff

We are looking for a clinician who is confident working autonomously while remaining closely integrated within the practice team.

Essential:

  • Registered healthcare professional (e.g.Paramedic, Advanced Nurse Practitioner, or Physician Associate) with relevant professional registration.

  • Experience inprimary care, urgent care, or community care.

  • Strongclinical assessment and decision-making skills.

  • Excellentcommunication and patient-centred care skills.

  • FullUK driving licenceand access to a vehicle. Knowledge ofWelsh NHS primary care systems.

  • Desirable
