Job summary

To work under the direction and supervision of the GPs, Practice Nurses and Practice Manager and in accordance of practice guidelines and protocols. You will assist the practice clinical team in the provision and delivery of patient care. Certain duties may require further training, which will be provided by the practice to the right candidate.

Main duties of the job

Conducting blood pressure, temperature, pulse checks, phlebotomy, ECGs and urine testing

Assisting with clinics for long-term conditions and conducting new patient health checks

Administering vaccinations as per protocol and undertaking BMI checks

INR testing using INRStar software

Assisting GPs undertaking minor surgery procedures and implants

Maintaining clinical records

Managing stock, treatment room cleanliness and complying with infection control standards

Daily recording of vaccine fridge temperatures

We are a modern skill mixed progressive team serving a population of around 8700 patients with 2 GP Partners, 2 Salaried GPs, 2 long term locum ANP's, 2 Practice Nurses and an excellent administration support team. We use EMIS Web and have access to Lexacom digital dictation.

We have easy and fast access to the M56, M53 and A55 so are easily accessible from Cheshire, Merseyside, Manchester and North Wales. We are uniquely localised between two nearest hospitals providing secondary care services, Wrexham which falls under NHS Wales, and Chester which falls under NHS England.

HCSW Job Description

This Job Description reflects the current position, and the post holder should expect that it may be subject to variation as the needs of the practice change.

Clinical

New patient registration checks

BP monitoring

Pulse rate and rhythm

Temperature

Urinalysis and preparation of specimens for investigation by the pathology laboratory

Phlebotomy

Weight monitoring including BMI

Offer health promotion advice/intervention as required

Minor dressings

Remove sutures, clips

Perform ECGs

Ensure any specimens are labelled and bagged ready for collection with the necessary forms

Administer influenza, pneumococcal and vitamin B12 vaccinations as per protocol

Assist with vaccine and cold storage including the monitoring and recording of fridge temperatures

Chaperone for clinical examinations, procedures

Monitor stock levels and order supplies

Restock and tidy GPs and treatment rooms

Report and discuss condition of patients causing concern with the practice nurses or GPs as appropriate

Ensure safe disposal of used and contaminated materials and follow infection control as per protocol

Ensure accurate completion of all necessary computer entries, other documentation associated with patient health care

Administration

Assist with recall and implementation for any additional work requested

Book HCSW/PN appointments for patients as and when required

Participate in audits if required

Make effective use of the training to update knowledge and skills

Using infection control procedures, maintaining work areas in a tidy, clean and safe way, free from hazards. Initiate remedial and corrective action where needed or escalation to Practice Nurses, Practice Manager

Promote and maintain good hygiene

Safe management of sharps procedures including training, use storage and disposal
