Practice Nurse(Experienced)

Newtown Medical Practice is a friendly, forward-thinking GP surgery providing high-quality care to over 13,500 patients. Based in modern, purpose-built premises, we work with the latest clinical systems including EMIS Web and Docman 10.

We are seeking a highly experienced and motivated Practice Nurse to join our supportive multidisciplinary team. This is an excellent opportunity for a confident clinician who enjoys working autonomously while delivering high-quality, evidence-based care.

The role is suited to a nurse with significant hands-on experience in primary care, general practice or community nursing, who is comfortable managing patients independently while contributing to a collaborative clinical team.

Main duties of the job

Main Duties

  • Chronic disease management (e.g. diabetes, asthma, COPD)

  • Assessment and treatment of minor illness, minor ailments, and injuries

  • Clinical assessment, diagnosis, treatment, and evaluation of care

  • Preventative healthcare and health promotion

  • Safe clinical decision-making and contribution to policy implementation

  • Collaborative working within the general practice team

They will also contribute to population health management, supporting initiatives that improve patient access, reduce health inequalities, and enhance health outcomes.

  • Registered Adult Nurse with the Nursing and Midwifery Council (NMC)
  • Minimum 5 years clinical experience in primary or community care
  • Recognised Practice Nurse qualification (degree level)
  • Experience in chronic disease management and minor illness
  • Ability to work autonomously within a multidisciplinary team

Desirable

  • Post-registration professional development in primary care, long-term condition management (e.g., asthma, diabetes), immunisation, cervical screening, etc.
  • Chronic disease management courses or certificates.
  • Independent nurse prescriber qualification
  • Additional specialist practice nurse experience or training
