Practice Nurse Oak Tree Surgery
We invite applications from the newly qualified ( Band 5) to experienced nurses ( Band 6) and our salary range of £31 516 - £41 437 reflects that. Our pay scales closely mirror A4C scales. We will consider applicants for a Job Share as well as a candidate seeking a full time post.
Oak Tree Surgery is seeking a motivated and compassionate Practice Nurse to join our friendly primary care team. This role involves delivering high-quality nursing care within a busy GP practice, including chronic disease management, immunisations, cervical screening, wound care, and health promotion.
You will work collaboratively with GPs, healthcare assistants, and administrative staff to support patient wellbeing and preventative care, while contributing to quality improvement within the practice.

Registered Nurse with current NMC registration
Strong communication and clinical skills
Primary care experience desirable but not essential

Main duties of the job
Reporting to our Nurse manager, your responsibilities will include:
Delivering chronic disease management (e.g., diabetes, asthma, COPD) -training provided
Cervical cytology screening
Childhood and adult immunisations
Wound care and dressings
Health promotion and lifestyle advice
Supporting QI and preventative health initiatives
Contributing to clinical audits and service development

Job Summary
The Practice Nurse is responsible for delivering high-quality, evidence-based nursing care to patients registered at Oak Tree Surgery. Working as part of a multidisciplinary primary care team, the post-holder will provide holistic care, support long-term condition management, promote health and wellbeing, and contribute to quality improvement initiatives within the practice.
Key ResponsibilitiesClinical Duties
Deliver chronic disease management clinics (e.g., diabetes, asthma, COPD, hypertension).( training provided)
Administer childhood and adult immunisations in accordance with national guidance.
Undertake cervical cytology screening.
Provide wound care management, including dressings and suture removal.
Carry out health checks and other preventative health screenings.
Perform travel health assessments and vaccinations (where trained).
Provide lifestyle advice and health promotion interventions.
Recognise and respond appropriately to safeguarding concerns.
Maintain accurate, contemporaneous patient records in line with professional standards.
Assist in delivery of research projects ( training provided)
Patient Care & Communication
Deliver patient-centred care that respects dignity, diversity, and confidentiality.
Provide clear health education and self-management support.
Communicate effectively with patients, carers, and the wider healthcare team.
Support patients in shared decision-making about their care.
Quality & Governance
Contribute to QI projects and enhanced service delivery.
Participate in clinical audit and quality improvement initiatives.
Maintain compliance with infection prevention and control policies.
Work within NMC Code and professional scope of practice.
Participate in significant event analysis and learning processes.
Team Working
Work collaboratively with GPs, healthcare assistants, pharmacists, and administrative staff.
Support mentoring and supervision of junior staff or students (where appropriate).
