The Role

We are seeking a motivated and compassionate Practice Nurse to join our clinical team. The successful candidate will play a key role in providing high standards of nursing care, health promotion, and chronic disease management.

Key Responsibilities

Undertake routine practice nursing duties including:

Chronic disease management (e.g. asthma, COPD, diabetes)

Immunisations and vaccinations

Cervical screening

Wound care and dressings

Health promotion and lifestyle advice

Support GMS contract, Qi projects and enhanced services delivery

Work collaboratively within the multidisciplinary team

Maintain accurate and timely clinical records

Adhere to NMC standards and practice policies

To provide high-quality, patient-centred nursing care within a GP practice, supporting the prevention, diagnosis, and management of acute and long-term conditions in line with NHS Wales policies and NMC standards.

Key Duties and Responsibilities

  • Deliver general practice nursing services including wound care, dressings, injections, venepuncture, and infection control.

  • Manage and run chronic disease clinics (e.g. asthma, COPD, diabetes, hypertension, coronary heart disease).
