The Practice is looking for a motivated individual to join the team at De Winton Field Practice, the role will include managing the repeat re-authorisations , medicines reconciliation from Hospital discharge etc , undertaking medication reviews . Providing advice to the practice clinical team, involvement with QI initiatives as appropriate.
There will also be a requirement to support the team with chronic disease management, nursing home patients , shared care medication and DOAC reviews/ initiation.
Main duties of the job
Clinical medication reviews, medicines reconciliation, chronic disease management, de-prescribing, repeat reauthorisations, patient queries .
About us
De Winton Field Practice is a training practice with a current list size of 9300 patients. Current staff includes 4 GP partners, 4 salaried GPs , cluster pharmacist, a pharmacy technician , an established nursing team, onsite physio service and mental health nurse. We aim to provide high quality primary care services and are located in a refurbished Health Centre. The practice is located approximately 40 minutes from Cardiff and has good transport links.
