The Advanced Clinical Practitioner, ACP works at an advanced level of clinical practice, providing expert patient care including assessment, diagnosis, treatment, and management of patients with complex needs. Working autonomously within their scope of practice, the ACP supports the delivery of high quality care, helps reduce GP workload, and improves patient access.
Main duties of the job
Independently assess, diagnose, treat and manage patients presenting with undifferentiated and undiagnosed conditions.
Manage acute, urgent, and chronic health conditions in patients across the lifespan.
Manage patients with long term conditions such as asthma, diabetes, COPD, and hypertension.
Prescribe medications, independent prescriber status essential.
Undertake clinical procedures for example, joint injections, wound care, ear syringing.
Provide urgent care for minor illness and injuries.
Contribute to QOF targets, chronic disease monitoring, and NHS Wales enhanced services.
Support home visits and care of housebound patients.
Deliver health promotion and disease prevention advice.
Recognise when referral to a GP, secondary care, or specialist is required.
