As part of our multidisciplinary workforce including GPs, Advanced Practitioners, Palliative/Frailty Nurses, Minor Ailments Nurses, Pharmacists, and Physician Associates you will play a key role in delivering high-quality, accessible care to our patients.

About the Role:

Main duties of the job

The ANP works autonomously within their professional scope to deliver high-quality care via face-to-face consultations, telephone advice, and home visits. They will exercise clinical judgment within their competencies to provide expert care, advice, and appropriate referrals, ensuring patient safety and best practice at all times. The ANP will independently manage patient caseloads, making clinical decisions on treatment, investigations, referrals, admissions, and discharge.

Key ResponsibilitiesClinical Practice

Deliver advanced nursing care as part of a multidisciplinary team, using expert clinical skills and evidence-based guidelines (e.g. NICE).

Triage, assess, diagnose, treat, and refer patients appropriately, both in-practice and through home visits.

Prescribe and review medications, request investigations (e.g. X-rays, bloods), and develop patient-centered care plans.

Coordinate care delivery, ensuring patient and carer involvement.

Refer patients to secondary care or other agencies as needed.

Communicate sensitive information with compassion to patients, carers, and families.

Provide specialist clinical advice and care plans, including fast-track referrals.

Contribute to national targets (e.g. QOF, QI Projects) and monitor care outcomes for continuous improvement.

Health Promotion

Lead health screening programs and innovate to meet health priorities.
