Job responsibilities

• Provide routine and same-day consultations for acute and chronic health issues

• conduct face-to-face and telephone consultations

• perform home visits where appropriate - very rare at the practice

• maintain accurate and timely clinical records using EMIS

• participate in clinical audit, QOF and practice-led quality improvement initiatives

• work as part of the wider multidisciplinary team

• Support the management of long-term conditions, medication review and preventative care

• manage patient correspondence, pathology results and referrals

• Contribute to duty doctor and on call rota

Experience

Essential

• Strong clinical knowledge and sound decision-making
• Excellent communication and interpersonal skills
• committed to holistic, patient-centred care
• ability to work independently and collaboratively within a team
• Good time management and organisational skills

Desirable

• Experience in chronic disease management, womans health
• Familiarity with QOF and NHS pathways
• Working knowledge of local services and referral pathways
