We have an exciting opportunity for an experienced Practice Nurse to join our welcoming and supportive practice nurse team. We are an independent team of 4 GPNs and 1 HCSW in a training practice with 3 GP partners and approximately 6500 patients. We take 3rd year nursing students and have an onsite pharmacist who also trains students. Hours of work and salary will be discussed at interview.
We have a strong practice team who work hard to deliver excellent patient care. You will work collaboratively within the general practice team to meet the needs of the patient, support the delivery of policy and procedures and provide nurse leadership as required.
You will undertake a mixture of general nursing clinics and chronic disease management
Main duties of the job
To provide an effective, quality driven Practice Nurse Service. To provide a high standard of general nursing care including health promotion.
To Patients a. Provide Specific Chronic Disease Management Clinics Clinics are run to provide the best possible advice to patients with chronic diseases and follow agreed clinical protocols These include Asthma, COPD, Diabetes and CHD. The successful candidate will be required to train for this if certificates not already held.b. Provide General nursing care including:- wound care removal of sutures request pathology tests hypertension management ordering of vaccinations to maintain stock levels health promotion vaccinations c. Carry out New Patient Registration examinations on new patients d. Perform ECGs e. Venesection (not routine) f. Travel & childhood Vaccination g. Provide holistic family planning health service to women including advice on all methods of contraception including LARC assess and administer injectable contraception offer opportunistic sexual health screening and ensure patients are advised and treated appropriately provide assessment and sampling for cervical cytology

Update both computer and manual case notes, if appropriate, after each patient consultation and minor operations. Input medical data onto computer from daily mail c. Summarise patients medical notes
About us
The practice was established in July 1984 and originally located in Spilman Street. In April 1991 we moved to new purpose built premises to offer up-to-date facilities. The surgery is located at the far end of St. Peters car park behind the bowling green. We are close to the main bus routes and parking is available in front of the surgery. We aim to provide a caring, efficient and professional service of the highest standard. In February 2004 the surgery became an approved training practice for the training of GP Registrars doctors who have undertaken their general medical training and wish to train to become GPs. We are a three partner and two salaried GP practice. We also have long term locum GPs as part of our team plus a highly skilled and dedicated nursing team. We have recently employed a Pharmacist to enhance the services we offer. Our receptionist and admin team are all dedicated and highly trained.

TO PRACTICE NURSES AND ALL DOCTORS FOR MEDICAL AND CLINICAL DETAILS Role and Responsibilities Clinical Practice: 1. Practice independently in accordance with UKCC code of conduct and with reference to Practice Guidelines, appropriately referring to GPs 2. Maintain accurate documentation of care and utilisation of I.T. 3. Ensure continuation of own personal and professional development facilitated by utilisation of clinical supervision networks. 4. Provide focus for nursing expertise within Practice, liaise with other nursing staff. 5. Identify training needs actively promote life-long learning. 6. Share responsibility for ensuring effective inter-Practice communication 7. Ensure nursing skills are appropriately and effectively used to deliver patient care. 8. Ensure care is delivered by Health Care Assistant with relevant and appropriate skills. 9. Ensure that efficient and appropriate stock control is in place. 10. Actively participate in educational programmes for whole Primary Health Care Team, including medical students and GP Registrar, in relation to areas of clinical and practice nursing expertise.
