Are you an experienced Pharmacist, ideally with a prescribing qualification? If so, we would really like to hear from you with a view to joining our friendly and highly skilled multidisciplinary team. Nant Dowlais Health Centre is a new GP practice serving a 10,000 patient list in West Cwmbran. Led by a multidisciplinary partnership, we are seeking a Practice Pharmacist with excellent clinical knowledge and experience to be a key member of our senior team, and also work closely with the GP Prescribing Lead to lead the medicines management agenda within the practice.
Main duties of the job
To improve the quality and effectiveness of medicines management
To provide patient focussed, prudent healthcare
To promote medicines safety and review polypharmacy
To develop the practice pharmacist role, becoming a core member of the practice team working effectively with other health and social care professionals delivering services to patients
Responsibility for leading and delivering a specialised pharmacy service as a key member of the senior practice team

This is an exciting time to join Nant Dowlais as we strive to develop our service using quality improvement methodology. The successful candidate will be enthusiastic and self-motivated with a positive approach to lead on agreed clinical areas according to competencies, to also include medication reviews with patients. You will join an experienced team which holds regular team meetings and educational sessions, and you will be expected to present on medicines management aspects of general practice. It is desirable to have good knowledge of the relevant areas of the GMS contract including QAIF, and also CEPP and other Health Board medicines management programmes. The role includes line management of Repeat Prescribing Clerks. We will support you to develop your role, working in an environment that nurtures and develops quality.
