Job summary

Hourly Rate: £11.09

25 to 37.5 hours a week

We currently have 2 vacancies: One for 37.5 hours per week and one for 25 hours per week to join our busy reception team.

Our surgery hours are between 8:00am to 6:30pm and the shifts worked will include either an early start or a late finish (negotiable).

Main duties of the job

We are seeking staff to join our friendly team in providing an excellent service to our patient population and ensuring they have access to our services in the most efficient and effective way possible.

The right candidate will have excellent communication skills and be able to show empathy and consideration, with a professional and courteous manner.

About us

Meddygfa Gyffin and Llys Meddyg Surgeries, based in Conwy, are part of the Llanfairfechan Group Practices.

Our highly skilled clinical team comprises of GPs, Advance Nurse Practitioner, Practice Nurse, HealthCare Assistant, Phlebotomist, Physiotherapist, Occupational Therapist and Pharmacy Tech. The clinicians are supported by our Care Navigation and Admin teams who work across both sites in Conwy.

Job responsibilities

The following are the core responsibilities of the Care Navigator. This list is not exhaustive and may be changed in order to provide a full service to our patients, which will be dependent upon workload and staffing levels.

  • Answering telephone calls from patients, carers, other NHS departments and deal with queries as appropriate.

  • Maintaining the electronic submissions of patients requests and queries.

  • Arranging appointments with the nursing team

  • Checking patients in for their appointments

  • Signposting patients to the correct services

  • Inputting data to patient healthcare records as necessary

  • Processing prescriptions

  • Managing e-mail queries effectively

  • Apply practice policies, standards and guidance

  • Maintain a clean and tidy working area
