Main duties of the job
Job summary:
To provide testing and assessment of patients on Oral Anti-coagulation treatment using point of care testing equipment, and to liaise with patients and GPs regarding patient dosing.
To assist medical personnel in the care of practice patients, to include treatment, preventative care, screening and patient education. The post will also include some administrative duties.
A uniform must be worn at all times, which will be supplied by the Practice.
They will act as a focal point of communication between patients, doctors, nurses and other members of the primary health care team

Fforestfach Medical Centre is a well established GP Practice situated in the heart of Swansea, close to the Gower Peninsula. Located in an area of economic growth and with a diverse population. The group itself is made up of two Partnerships; Sartori & Partners and Bensusan & Partners, each with individual patient lists. We have 10 Partners, 4 salaried GP's and have a supportive clinical team of Practice Nurses, HCAs and an INR HCA, with excellent admin and Management support. We are part of Penderi cluster with a growing MDT and patient services. We are a highly respected teaching Practice for GP Registrars and Medical Students. Many of our Partners have gone on to work for us after training. Our growing practice list sizes are 7,600 and 6,600 respectively.

Duties and responsibilities:
1. Nursing
Main Duties will be to support and organise INR POC Testing service for patients on oral-anticoagulation. This is a new service within general practice commencing from October 2017. Full training will be provided.
You will be expected to perform INR testing and consult with the patient, input data into the designated computer programme, and advise patients of the dosing as per the clinical software.
Arrange follow up appointments as recommended by the clinical software and/or GP.
Liaise with a designated GP regarding recommended dosing in complex patients.
Recognise any changes in the patients observations and report and act upon, liaising with the Doctors.
Maintain a register of patients and completed necessary admin work.
Maintain stock levels.
In addition to the above usual HCSW duties include:
New patient registration checks (where necessary) and routine patient checks.
Taking measurements for; pulse, temperature, peak flow etc.
Weight monitoring
BP monitoring
Urinalysis
Phlebotomy
Performing ECGs.
Provide lifestyle advice to patients making any necessary referrals within the practice
Ensure specimens are labelled and bagged ready for collection with the necessary paperwork completed.
Ensure clinical waste collection runs smoothly, reporting any problems to the nurse manager.
Ensure sharps bins are ready for collection.
Restocking/maintenance of equipment.
Restocking of clinical areas and consulting rooms.
Recognising changes in the patients observations which must be reported and acted on.
Assist clinicians in emergency management of patients.
Chaperone duties.
Supporting minor operations and procedures.
Administration and professional responsibilities
Participate in the administrative and professional responsibilities of the practice team
Ensure accurate and legible notes of all consultations and treatments are recorded in the patients notes
Ensure the clinical computer system is kept up to date with accurate details recorded.
Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice.
Ensure collection and maintenance of statistical information required for regular and ad hoc reports.
Ensure correspondence, reports, results etc are filed in correct records
Attend and participate in practice meetings as required.
Assist in formulation of practice philosophy, strategy and policy.
Ordering stick, restocking and maintenance of clinical areas and consulting rooms.
To create and send recall letters for clinics.
To assist in seasonal and special projects as requested e.g. flu campaign.
Ordering and display of health promotion materials.
Follow infection control standards.
3. Management of appointment system
Ensure total familiarity with all appointment systems, including regular and incidental variations.
Book appointments and recalls, ensuring sufficient information is recorded to enable retrieval of the medical record.
Monitor flow of patients into the waiting room, ensuring the appointment system accurately reflects the arrival of patients.
Monitor effectiveness of the system and report any problems or variations to the HR Lead or senior receptionist.
