Job summary

The prescribing clerk will be part of the team responsible for the accurate and timely processing of prescription enquiries for the patient population, liaising effectively with all relevant personnel. This role is not just about printing repeat prescriptions. The role requires someone who is administratively strong, who has determination, persistence and initiative, and who can work within clear guidelines in order to be able to deal with more complex enquiries and pull together sufficient evidence for the prescribers.

Job responsibilities

Job Description

The following are the core responsibilities of the Prescription Clerk. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

a. Support the practice clinical pharmacist as required

b. Deal with all prescription related enquiries from patients as a first port of call and in a timely manner

c. Process acute and repeat prescription requests received by the practice, by phone, email, post, slips and the online access system, in accordance with practice protocol and in a timely manner

d. Accurately reconcile medications as per discharge notes and letters from the hospital

e. Change medications on the clinical system as advised by secondary care or other multi-disciplinary clinicians, and put prescription in for signing by a prescriber with accompanying evidence

f. Support enhanced service work, especially shared care and DOACS

g. Produce prescriptions for the GP to sign in an acceptable timeframe

h. Liaise closely with the prescribing hub to coordinate processes for benefit of patients

i. Generate requests for medication on behalf of the patient for non-repeat items

j. Actively encourage patients to reconcile medications to facilitate monthly collections

k. Complete prescribing cost-saving initiatives as directed

l. Discuss with patients their requirements, encouraging patients to order only what they require

m. Act upon compliance issues, liaising with the appropriate clinician

n. Ensure medication reviews are arranged by maintaining an accurate recall system

o. Effectively liaise with external services, i.e., district nurses, to ensure medicaments are arranged for housebound patients

p. Input data into the patients healthcare records as necessary

q. Record and report adverse effects accurately and appropriately, informing the practice manager immediately

r. Data input onto the clinical system

s. Carry out system searches as requested

t. Maintain a clean, tidy, effective working area at all times

In addition to the primary responsibilities, the Prescription Clerk may be requested to:

a. Partake in audit as directed by the audit lead

b. Support the practice management team in reviewing and updating the organisations prescribing policy

c. Complete opening and closing procedures in accordance with the duty rota
