Job summary

An exciting opportunity has arisen for a highly motivated Practice Nurse to join our experienced multi-disciplinary team at Redlands Surgery Penarth. The role is for 21 Hours allocated to a Wednesday, Thursday and Friday. The Practice Team currently consists of 2 GP Partners, 4 Salaried GPs, 2 Practice Nurses, 2 HCSWs,1 Clinical Pharmacist and a very supportive Administration/Reception Team.

We are a friendly Practice with a patient population of 9350.

Experience in a similar role is desirable

Main duties of the job

The role is integral to our nursing team and will involve undertaking a wide range of Practice Nurse duties including Chronic Disease Management, Anti-Coagulation Management, Cervical Cytology, Travel Vaccinations, Childhood Immunisation and Treatment room duties.

We are a well-managed and organised practice, established within the Eastern Vale Collaboration (EVC) of Cardiff & Vale University Health Board. Currently we have a list size of circa 9,400. One of our partners is the Vale Constituency Lead for the Bro Taf LMC. We provide a full range of enhanced services to our patients including minor surgery, LARC/Coil contraception, nursing home enhanced care and shared care monitoring. We currently have 2 partners and 4 salaried GPs who between them provide 33 sessions, 2 HCSW's and currently 2 part time nurses equivalent to 1.33 WTE.

JOB TITLE: PRACTICE NURSE

Band: 5-6 AFC

Reports to: Lead Nurse and GP Partners (clinically)

Practice Manager (administratively)

Hours: 21 on a Wednesday, Thursday, Friday

Annual leave: 5 weeks pro rata per annum plus Bank Holidays .

Job Summary: To provide treatment room services to the practice population by evidence based, quality focused care. To assess, plan, implement and evaluate care. To ensure clear channels of communication exist between clinical, administrative and community teams. To progress the role of the practice nurse in primary health care and contribute to both national and local service development and initiatives.

Teamwork: All employees are expected to display a high standard of teamwork. This includes cooperating in undertaking the work of absent colleagues or assisting others where the workload is particularly high. Accordingly there may be a requirement from time to time, to undertake other duties.

Responsibilities:

To provide assessment, treatment, screening and health education services and advice, working in accordance to set protocols, guidelines and NMC Code of Conduct

Provide nursing service/treatments to patients independently or in participation with General Practitioners with reference to aforesaid agreed protocols and guidelines

Professional Responsibilities:

To be a Registered General Nurse with current NMC registration

To undertake reflective practice and regular performance review in accordance with the NMC recommendations for revalidation of clinical practice

To effectively manage own time, workload and resources, acknowledging limitations to own clinical practice/knowledge and in the delegation to others. Recognising when referral to other health professions is needed

Evaluating and adapting care using evidence based practice in consultation with colleagues as a forum for team education, development and audit

Using evidence based practice to assist in the formulation of practice philosophy, strategy and policies

Specific Duties:

Nursing procedures to include, but not limited to:

Public Health disease prevention, to include adult and childhood immunisation, seasonal and travel vaccination. This will include seasonal visits to housebound patients for administration of influenza/pneumococcal vaccination

Chronic disease management.

Immediate and necessary clinical intervention such as venepuncture, ECG monitoring, Hydroxocobalamin administration and sample testing.

INR monitoring

Cervical Cytology

Other Duties:

Working in accordance with the NMC Code of Conduct:

Concise, accurate record keeping and documentation with competent utilisation of the Vision software

Promotion of confidentiality and patient dignity with protection of personal and sensitive data

Participation within clinical audit/research and the monitoring of standards of care

Participation in the education of students of all disciplines

To act as an educational resource for other nurses, health care professionals and non-clinical staff within the multidisciplinary team

To adhere to strict Infection Control in line with policies and guidelines (ANTT)

Maintenance of treatment room supplies with specific reference to maintenance of the cold chain and stock levels of vaccines.

Maintenance and checking of stock equipment such as resuscitation equipment and emergency medication accompanying the equipment

Any other duties as reasonably requested by senior staff, inclusive of GPs and Practice Manager, providing those duties are within the scope of the role and that the individual is trained and capable of such duties.
