Job summary

We are creating a new and dynamic GP Partnership and have recruited a new Management Team to develop and create friendly and forward-thinking GP Practice located in the Rhondda Valley. Our main surgery is in Ynyswen and we have branch sites in Tynewydd and on the High Street, Treorchy. Staff may need to work across all three sites. We currently have 3 GP partners and approximately 16700 patients. We have our own Pharmacy Team, Advanced Nurse Practitioner, Advanced Paramedic Practitioner, Advanced Mental Health Practitioner, Physician Associate as well as support from a fantastic nursing team including Practice Nurses, Chronic Conditions Nurses, Health Care Assistants and phlebotomist. We have exciting plans for the future and are looking to recruit similarly minded individuals who can engage with patients, empathise with the needs of our registered population and represent the best of our valley community.

We are looking for confident Care Navigators to join our extremely busy, friendly, dynamic GP Practice. You will need excellent communication, organisation and IT skills, a friendly disposition and the ability to remain calm under pressure. You will need to be flexible and previous experience of working in a health care setting would be an advantage.

We have Full Time and Flexible Part Time working opportunities available.

Main duties of the job

Receive, assist and direct patients in accessing the appropriate service or healthcare professional in a courteous, efficient and effective way.

Provide general assistance to the practice team and project a positive and friendly image to patients and other visitors, either in person, via the telephone or using other electronic means.

You will be using a new Care Navigation system to help you navigate patients to the most appropriate care at the first point of contact. This may be an appointment within the surgery with one of our clinical staff, or signposting to another NHS service such as Physiotherapy, Community Pharmacists, Social Prescribing, etc. Practice staff have had (and will continue to have) significant involvement in the design of the system, and we believe it will be a huge help to both new and existing staff in dealing with constant changes in NHS services.

This is an opportunity to join a circa 17,000 patientPractice which currently operates over three sites in Treorchy and Treherbert within the North Rhondda Cluster. You will be part of a multi-disciplinary team that includes GP Partners, Salaried GPs, Advanced Nurse Practitioner, Advanced Paramedic Practitioner, Physicians Associate, Mental Health Practitioner, Pharmacists and Physiotherapists. The Practice is committed to multi-disciplinary team working in its delivery of healthcare to the community.

The practice is a training establishment for GP registrars and Foundation Pharmacists and has close links with HEIW and CTMUHB due to the GP Partners other commitments. Participation in weekly Clinical Practice Meetings would be encouraged the MDT regularly meets to discuss shared learning, audits, prescribing advice, significant event analysis and quality improvement processes and practice. The professional development of individuals is recognised and championed including opportunities to extend scope of practice or progress a specialist interest in an area.

You will be fully supported by our GPs & practice management team and as a patient-focused practice, in-turn you will be asked to deliver the highest quality service to our patients and our practice community
